Requests for Medical Maggots and maggot therapy supplies can be placed by phone, fax or e-mail, using any of the convenient order forms listed below. Note: Medical Maggots require a doctor's prescription. You will be asked not only for the doctor's contact information and e-mail address, but also for DEA # or other evidence of prescribing authority.


A copy of the Package Insert is available for viewing or printing.


Terms and Conditions Medical Maggots (disinfected Phaenicia sericata larvae) are regulated by the FDA as a prescription only, single use medical device. They must be ordered by a medical practitioner licensed to prescribe wound care treatments. Read all package labeling. Problems with product quality must be reported within 24 hours of receipt to be eligible for refund or replacement. Maggots are prepared upon order; they are highly perishable and cannot be stored, reused, or returned. A fee may be assessed for orders cancelled less than 24 hours before shipping. Maggots are provided only for Patient Care; this is not a license to breed or redistribute them. The ordering clinician assumes full responsibility for patient care. No promise of safety or efficacy is implied nor should it be inferred. Prices subject to change without notice.


Refund/Return Policy Medical Maggots are prepared upon order and are highly perishable; they cannot be stored, reused, or returned. Problems with product quality must be reported within 24 hours of receipt to be eligible for refund or replacement. A fee may be assessed for orders cancelled less than 24 hours before shipping. Maggots which arrive dead, or more than 6 hours late, Tuesday through Friday, will be replaced or refunded without charge, if they cannot be used and if the customer completes the required documentation (simple questionaires, required for quality assurance and subject to FDA review).

If Medical Maggots ordered for Saturday delivery arrive dead, they will also be be replaced or refunded without charge, according to the same provisions. However, since Monarch Labs does not advise Monday deliveries (due to their extended transit time) orders for Monday deliveries will not be replaced or refunded.

Sorry, we cannot refund or replace orders with shipment problems resulting from incorrect addresses furnished by client, or because no one was available to take receipt of the package, or because of other customer errors.
